When should I use Assured RX Home Delivery Prescription Service?

The program is designed mainly for individuals using maintenance type medication for the treatment of chronic or long-term conditions such as, but not limited to, diabetes, arthritis, heart conditions or high blood pressure for periods longer than 30 days.

 

What is covered?

 

The program covers any drug, which by federal law requires a doctor's prescription, and is covered by your prescription benefit program. Insulin and insulin syringes require a prescription when ordered through Assured Rx. The program does not cover medication that can be purchased over the counter or is excluded by your plan sponsor. The plan sponsor may supply some over the counter medications directly. Please contact your plan sponsor member services for further details.

 

What about generic drugs?

 

The generic name of a drug is simply its chemical name. Generic drugs meet strict FDA requirements and are as safe and effective as brand name drugs, but considerably less expensive. Unless otherwise specified by you or your doctor, your prescriptions will be filled generically if legally available in that form. Some plan designs allow for an additional copay amount for brand name orders where a generic equivalent is available. Please contact your Plan's Customer Service for co-pay information before ordering your prescription.

Is my information kept confidential?

 

In order to safely and effectively monitor your prescription drug therapy and better serve you, we have requested information such as date of birth, medical conditions and known drug allergies. This information, as well as all personal information, is held in strict confidence. Assured Rx will only use this information to provide you with the highest level of pharmacy care. Please read the enclosed Privacy Notice, and sign the space provided on the order form to acknowledge you have been provided the Notice of Privacy Practices.

How do I use the program the first time?

 

Five Easy Steps:

  1. When prescribed a maintenance drug, have the
    prescription written for the mail order days supply set by your
    plan (for example, 90 days). By law, Assured Rx can only
    fill the quantity indicated by your doctor.
    Note: If you need medication immediately, have your doctor
    write two prescriptions: an immediate supply for your local
    pharmacy, and a second to be mailed to Assured Rx.
  2. Complete the Enrollment Order Form and Confidential
    Patient Profile for you and any covered family members
    ordering prescriptions. This form will need to be
    completed for your first order only. Changes should be
    submitted to Assured Rx in writing.
  3. Mail the completed form, along with the original
    prescription(s) and co-payment, in the envelope provided.
  4. Be sure to write patient name and member identification
    number on the back of each prescription.
  5. Orders are delivered postage paid to the address
    provided on the form or anywhere in the US. 

How do I refill or order new prescriptions?

 

Refill (Three Options):

  1. Complete the form provided with your order, enclose payment, and mail to Assured Rx; OR
  2. Call Assured Rx at 888-987-9977 with member identification number, prescription number(s) and credit card information. Note: The prescription label and invoice will indicate number of refills available and date prescription is eligible for refill; OR
  3. Visit our website, www.assuredrx.com

 

 

 

Am I charged for shipping?

 

Standard shipping is FREE on all orders containing prescriptions. Prescriptions that require special handling are sent according to delivery specifications free of charge. Overnight and second day delivery is available for an additional charge on standard prescription orders.

How soon will I receive my order?

 

Our goal is for you to receive your shipment within 10 days after placing your order. Please allow a few extra days when placing an order for the first time. If you have any questions or problems concerning your prescription order, or if you do not receive your order in 10 days, please contact Assured Rx at the toll free number provided.

 

Am I able to have my Medications delivered to an address other than my home Address?

Your medications can be mailed anywhere in the continental United States. Please note that for the next order, you must notify us of any additional shipping address changes.
